As Dave points out, it's quite common in Scotland. Lanarkshire boy, myself. Used it colloquially for many years without ever noticing it was "wrong". The intention of the word does not change if used singularly as a question ("How?", to mean "How come?"), though I'd never write "How?" by itself. I'd assumed it was just another peculiarity between Scottish dialects and formal written English.

the ones that particuarly wind me up....

"can i get you a .............. with that?" ie. being offered a carwash token when you buy petrol..
"would you like a meal deal?"
"would you like a drink with that?"

If i want something else Ill bloomin ask for it!!!!!!!

Yeah, in Superdrug when you're just in for you conditioner and toothpaste, 'would you like any stamps or mobile top up with that?'. NO I DON'T :evil:

The tills in those stores prompt the cashier to ask those questions (for the large part), it can be good sometimes.....
